I was sitting in my study last night, just relaxing and scanning various
articles and mindless reportage on celebrities, just to stay in touch with
the younger set, when I came across a title on Wonderwall that screamed:
“Secrets of Lady Gaga’s New Liquid Diet.”
Well, that got my attention, which is the first job of any headline. Then,
I read further, which is the second job of a good headline.
Turns out that it was an article concerning the Grammy Award winning singer,
Lady Gaga. Now, I confess to a fondness for Lady Gaga’s songs (my favorites
include Poker Face, Papparazzi, Alejandro, Bad Romance and her newest,
Born This Way), so this story caught my attention.
I have to admit, when I first saw the Liquid Diet term, it dredged up a
whole host of bad memories of the horrible liquid protein diets that were
popular in the 1970′s. They were made from horse hides, smelled like
turpentine and tasted like cough medicine (though they supposed to taste
like cherry).
Worse than that, they were deadly in many cases. Some people who tried this
approach ended up dying, because of electrolyte imbalances. I tried it myself,
for a short time, which is how I know how bad it tasted. (Hey, I wasn’t always
a font of medical wisdom–this was before my training was complete. Besides,
it seemed like a good idea, at the time…)
The point is, these liquid protein diets were completely discredited as
being nutritionally incomplete, as well as being very dangerous. Not only
that, but after several days, your teeth begin to hurt, from the lack of
chewing. Just horrible. Anyway…
This isn’t that. It seems that Lady Gaga is fond of “blending.” The “blended
diet” referred to in the story is what we in the medical profession call
“full fluids,” when we order such a diet for patients. This includes what
old physical fitness pros, like Jack LaLanne, used to call “Juicing.”
If you’ve ever seen Lady Gaga perform, you know that, in addition to singing,
she burns a tremendous amount of energy, dancing, running across the stage,
climbing up and down stairs, and doing what amounts to acrobatics, for hours at
a time. And she’s a tiny, skinny little thing, without much body fat for energy
reserves.
So her diet needs to fuel high energy activities, while maintaining good
health. Not an easy order to fill.
She’s being advised by celebrity “trainer to the stars,” Harley Pasternak,
who is working with her on diet and exercise, to keep her fit and flexible
for an upcoming grueling concert schedule.
According to him, “She likes berries. She likes the fiber in berries. They
are low calorie, colorful and carry a lot of antioxidants, so we’ve been
doing a lot of creative, sweet things, with all berries like blackberries,
strawberries, boysenberries and cranberries. We use nonfat Greek yogurt
as a base and just go to town. We’ve also been doing a lot of pureed soups
like broccoli, cauliflower and asparagus.”
Now, you may not like the idea of blending exclusively for your meals, but
as an adjunct to your own diet plan, you could do a lot worse. Because they’re
already liquid, requiring little digestion, the blended meals are rapidly
absorbed, making their contained energy almost instantly available.
The berries provide a ton of fresh antioxidants, which, as I’ve told you many
times, quench free radicals and prevent cellular and DNA damage. The
absorption of sugar (fructose) from the berries is slowed down by all the fiber
they contain. That minimizes the surge of insulin and makes the concoction
functionally a much lower glycemic index meal than it would have been.

Ever since I began promoting this great taste treat, as a super source of

Wherever you look on the web, on TV, in magazines, or in the the papers
(especially the tabloids), there is always…I mean ALWAYS… a new diet
being promoted. You might reasonably ask yourself, “Why are there so many
new diets?”
The answer is really simple, though. People simply don’t DO them. They
read about them, they may even try to begin them, but they often don’t
stick with them long enough to make a difference.
That’s why there are so many “new” diets, usually associated with some
celebrity. The truth is, there are only a few basic principles that
underlie every successful diet.
Nobody’s creating new principles, right? I mean, the body only works the
way it works. Nothing has changed in a million years: physiological principles
are what they are.
That’s why there is only one fundamental truth, when it comes to weight
loss: ALL diets work…IF you DO them.
The differences are how healthy the diet is, how well you feel during the
period of deprivation, how easy is it to adhere to, how quickly and
efficiently does it mobilize and metabolize fat, and how well does it
integrate into your life, so you can remain on the program and maintain
the weight loss achieved, for the long term.
That’s basically it.
Now, if weight loss was easy, everyone would be thin, right?
The question that comes to mind is, WHY isn’t it easy?
The answer is that your system is designed to protect your body, in case
of a famine. In the event of food scarcity, metabolism shuts down to a bare
minimum, becoming maximally efficient for energy expenditure, while chemical
pathways change, so as to hold on to every scrap of fat.
In addition, muscle is catabolized, that is, it’s broken down and “burned”
for fuel.
This is why, in cases of true starvation, the victims look like animated
skeletons. Their muscles and fat have been digested for energy requirements.
If you’ve ever seen photos or movies of concentration camp prisoners, or
refugees from war torn regions, you will know exactly what I’m talking about.
Now, in western countries, true starvation is not the problem. But perceived
starvation IS. The body can’t tell the difference between real and apparent
starvation. So diets that dramatically cut back calories, trip the inbred
protective mechanisms that result in fat retention and muscle catabolism.
In the initial stages of the diet, these mechanisms also make you ravenously
HUNGRY. And that hunger prompts you to EAT, at least until your body begins
to produce ketone bodies, from fat oxidation, to burn instead of glucose.
Ketone bodies can be used by cells for fuel, and they also kill appetite.
That’s the main reason why I reject calorie restriction diets, like low fat,
low calorie diets. They make you hungry, they promote the loss of muscle
mass, and they gear DOWN your metabolism–all the effects which are the
opposite of what you want.
Now, other high protein diets, like Atkins, Sugar Busters, Protein Power,
South Beach and most recently, Dukan, save your muscles, kill your appetite,
and melt your fat. But they also shed a lot of water, and if prolonged, may
lead to nutrtitional deficiencies.

Well, it was back to the vet for a followup visit for two of my feline
brood last Friday. One little guy, Toby, is now past 15 1/2 years old,
pretty old for a cat. Poor boy had a bilateral myringotomy (a surgical
opening of the ear drums, usually for infection), some weeks ago.
That complicated an intial ear cleaning and senior dental treatment,
including a few tooth extractions for perodontal (gum) disease. As luck
would have it, he managed to get a complication–his lower jaw fractured
through one of the premolar extraction sites. That necessitated a prolonged
anesthesia while the vet did her best to fix the fracture with a wire.
When he came to, he wasn’t the same cat. It’s like he was dumbed down, with
a dull affect, periodically caterwauling with blood curdling screams. Lovely.
For some weeks, we tried supportive care, hoping he would improve witrh
time, but he got no better. Finally, we took him to the local veterinary
specialists. After the proverbial million dollar workup, CT scan, lab
tests, neuro exams and the like, he was referred to the dermatologist for
an ear evaluation.
Don’t ask me why, but Dermatology does the ear exams for this specialty group,
presumably because they own the fiber optic otoscope that allows them to
visualize the ear canals, which are difficult to see in a cat, because they
take a right angle, on their way into the middle ear.
Turns out he had a rip roaring middle ear infection (otitis media). So he
ended up with those bilateral myringotomies (surgical piercing of the
ear drums), to permit drainage of the middle ear and allow pus to get out.
We have been hoping that he would then recover, but so far, no such luck.
The other cat is Emily, also a geriatric, very vocal, needy, pretty little
black domestic short hair cat, with big, green-tinged eyes. That poor girl has
bad kidney disease. So for the past several months, I’ve been keeping her
going with daily fluids, delivered by a subcutaneous instillation of 100cc
Ringer’s Lactate, to keep her kidneys flushed.
I also have to give her capsules of aluminum hydroxide, so her serum
phosophorus level doesn’t get too high (because, if it does, she will lose
her appetite, stop eating and drinking and shortly thereafter, fade away
and die.
I’m telling you, I have a virtual cat clinic going on at home. And the bills!
There are no controls at all on what vets can charge. And they do. I’ve spent
enough to buy a small used car on these two alone. The month doesn’t go by
that two, or more, don’t need to see the vet, which adds up (though the local
vets are not nearly as bad as the specialists).
It’s ironic: people bitch about their doctor bills, all the time. But just
compare: a human surgeon (that is, a surgeon FOR humans) gets about
300 bucks for a simple appendectomy on Medicare, and that includes three
months of after care!
By contrast, I’m looking at thousands for the workup and treatment on a
ten pound cat AND they charge for followup visits, too!
In the Veterinary Ethic, the client (ie., the owner, or the one who pays)
determines the level of care provided to the patient (the animal), indeed
determines if that animal lives or dies, by euthanasia. In my case, a simple
euthanasia would cost me about 75 bucks…and a lifetime of heartache.
The Medical Ethic, by contrast, provides the best available care, regardless
of cost, the wellbeing and survival of the patient being the highest value.
And that’s how things used to be (and the way I was trained).
As Medicine becomes more and more controlled by rising costs, and third parties,
whether insurance companies or the government, assume the burden of the costs,
more and more restrictions will be imposed on the patients and their choices restricted
on the basis of cost.
So while I attempt to apply the Medical Ethic to my pets (at considerable cost),
the insurance companies and the government, more and more, attempt to apply
the Veterinary Ethic to people! At least with the insurance companies, if you
don’t like your treatment by one insurance company, you can always go to another.
But if your argument is with the government, ie., Medicare or Medicaid,
who you gonna call? You are S.O.L., pal! If you, like me, are getting close to
that magic age, when choice is deprived and you’re basically forced to sign
up, it’s a matter of some concern.
The “benevolent” despots of the Administration have slashed over HALF A TRILLION
from the Medicare budget in the Patient Affordability Act (ObamaCare). That’s
bad enough, but what really worries me is the spectre of rationing, which is
preordained and inevitable, if this law stands.
If I am healthy and vital, regardless of my age, I don’t want some pencil
neck geek bureaucrat determining that any treatment I might need is too
much to spend for someone “my age”…instead, I should “just take a pill,”
as our glorious leader once said. They made great fun of Sarah Palin for
talking about “Death Panels,” but when you get right down to it, that’s
exactly what we’re talking about, here: determining who gets what treatment,
largely based on their age and the cost.
If that doesn’t scare you, you are either ill informed… or an idiot. If it
does scare you, as it does me, then you are at least in full possession of
your faculties and have a realistic grasp of what we’re facing.
Here’s my advice, which, if you’re smart, you’ll take. If you’re not already
healthy, then get healthy, as fast as you can. Then, stay that way, indefinitely.
If you’re vital and healthy, you won’t need the system, whatever it becomes.
And that’s the only way to win–stay OUT of the system entirely, by never
needing to access it.
That means ditching the smokes and excessive drinks, or the illicit drugs,
or excessive eating, or living a sedentary lifestyle. Instead, start to eat
right, lose excess weight, especially belly fat, which is known to be associated
with many diseases, including heart disease, cancer, stroke, diabetes,
lung disorders, even Alzheimer’s.

Did you see the report about the guy that found a cache of some of the
rarest and most desirable collectable comic books in his great aunt’s
basement? Turns out the lot was recently auctioned off… at over 3.5 MILLION
bucks, this past Wednesday in New York, under the auspices of Heritage Auctions,
out of Dallas Texas! It’s been all over the T.V. news and the web.
Michael Rorrer, lucky owner of the cache, discovered the neatly stacked comics
in the basement closet of his great aunt’s Martinsville, Virginia home, after her
death, last year. The collection was assembled by his late great uncle, Billy Wright.
But after his death at age 66, in 1994, it had been lost in the basement of his home,
until Rorrer discovered the comics, during house cleaning.
This collection was remarkable not only for its discovery, but its quality. It included
five of the top six comics in the industry, and also 45 of the top 100 comics, overall
and all were unrestored (meaning, in their original, unretouched condition).
Some of the classic titles in his find were the first edition of Action Comics,
Superman’s first appearance, in 1938, and Batman’s first appearance
in Detective Comics #27, 1939. The latter, initially priced at only 10 cents,
brought over half a million at auction, all by itself. Superman’s debut
brought just a hair under 300 grand!
The amazing thing is that among comic book aficianados and collectors, the
apocryphal story of the “mother lode,” a batch of rare comics, hidden away
in someone’s basement, or in an attic, by someone’s mother is an evergreen
concept, part of comics lore. But that is exactly what has happened. Amazing!
Now, you may think that anyone who would pay over half a million for an
old comic book, a fragile, continually degenerating tangible asset, is a
friggin’ nut–and from an entirely rational point of view, you might be
thought correct. But that fails to take into account the passion of the
true collector, whether the collectables be Rembrandts… or comic books.
I remember when I was a teenager and I actually held in my own hands
the first issue of the Fantastic Four, or the Amazing Spider-man’s first
appearance, or my personal favorite character, The Mighty Thor, the God
of Thunder, who first showed up in Journey Into Mystery #83. Prices had
increased to 12 cents by then.
I was too young to have lived during the “Golden Age of Comics,” the late
1930′s and early 1940′s, when all the iconic comic characters, like Superman,
Batman and The Green Lantern, were first created.
But the early 1960′s were the dawn of the “Silver Age,” when the FF, Thor,
Spider-man, the Hulk, X-men, Iron Man, Captain America and the Avengers,
as well as a host of other highly imaginative characters, were created by
the fertile imagination and storytelling skills of Stan Lee and Jack Kirby,
the legendary creative geniuses behind what became Marvel Comics, one of
the “Big Two” comics publishers (the other being D.C. Comics, named for
the aforementioned Detective Comics).
The point was, I WAS around for that “Silver Age” period and I had all of those
classic issues when they were brand new. Every one of them, in my own hands.
And, just as in all the stories, my own mother managed to “clean my room,”
getting rid of all my “junk,” including all the comics I had collected, all those
valuable first editions (although I suspect that a number of my prized early
edition comics were “liberated” by my brother, while I was away from home
for my internship).
If only I still had those issues today….
So, like all true fan boys everywhere, I was thrilled for the guy who actually
found this treasure trove, The Billy Wright Collection… and cashed in on
it. It was like all those times we had believed in the possibility of that
“mother lode” story were finally vindicated.

Just today an article was published in the New England Journal of Medicine
(the “Bible” of Internal Medicine, here and abroad) that reported an amazing
finding. It seems that the removal of precancerous lesions during routine
colonoscopies has been demonstrated to slash the risk of dying from colon
cancer by HALF.
That’s a pretty big deal. And it’s more evidence of the value of yearly
maintenance for the human body.
That’s why I recommend yearly physicals, together with appropriate testing,
like colonoscopies every five years, beginning at age 50 (or earlier, like 40,
if there is a family history of colon cancer).
Also included are EKG’s, stress tests, ecchocardiograms, ultrasound exams
of the carotid arteries, if needed, and lab tests at least yearly, often up to four
times a year, for vitamin D3 levels, liver function tests (especially if you’re on NSAID
medications, or long term niacin, also called vitamin B-3, to lower LDL, or “bad,”
cholesterol–all of which can irritate, or adversely affect, the liver).
Chest x-rays are used to rule out occult lesions of the lungs and ribs, while
urinalysis can tell us in an instant if someone has full blown diabetes.
Blood tests rule out various forms of anemia, as well as infection, blood
diseases (like leukemia), markers for inflammation (CRP) and the tendency
of blood to clot (platelet levels, protime, partial thromboplastin time, bleeding
time).
A full chemistry panel (CMP, or Complete Metabolic Panel), measures blood
electrolytes, minerals, liver function tests, blood sugar, and all are needed as early
warning screens for a number of diseases like diabetes, or liver disease
Needed less often, but helpful once in a while, are blood tests for determining
levels of homocysteine, a byproduct of methionine (an amino acid) metabolism.
High homocysteine levels can be associated with heart disease, because these
higher levels can injure the endothelium, the inner lining of the coronary arteries.
This, in turn, leads to a “repair” process, in which cholesterol is deposited within
the artery walls. These plaques gradually narrow the inner diameter (lumen) of
the blood vessels.
Another, more rarely needed, test is lipoprotein (a), which is an independent
risk factor for heart disease. Your doctor would determine when and if this
test is needed.
The point is, just like your car works best if you are conscientious about
maintenance on a regular, annual basis, so too is your body more likley to
do better, when subject to regular examination and monitoring.
The other part of good maintenance is pre-emptive action. In a car, that’s
