Releases You From Knee Pain

I don’t know why I never thought of this before, but it seems like

Let me give you an example.

Especially when you’re talking about weight loss, or vigorous exercise,

I recognized my old enemy, chondromalacia patellae. This is a
degenerative softening of the cartilage behind the kneecap, which
eventually progresses to shredding and fragmentation. That shredding is what
causes the crunches we hear (and feel). It’s a “first cousin” of osteoarthritis.
And once you hear that characteristic grating sound, the knee will never be
“normal” again. The damage is done.

But there are still things you can do. I told this young man that he

He looked a bit skeptical, so I explained. See, he probably caused his
own problem by deep squats with heavy weights. Even in the young, that
exercise overloads the posterior horn of the menisci and the articular

those same structures, while stimulating strength and endurance in the
quadriceps muscles. But, no one teaches these young studs that simple
fact. So they end up like this guy.

The other day I was in the local vitamin store, looking for some of
the basics in my daily regimen, which were running low. While I was
there, a woman came in asking the clerk (or Health Enthusiast, as
his badge proclaimed him) where the calcium was.

I thought that she was probably looking for calcium supplements to
reinforce her bones, since osteoporosis is one of the banes of
post menopausal women (she was a pleasant looking middle aged woman,
just pre, or perhaps post-menopausal). But no, she mentioned to the
clerk that she was told she had arthritis, so she was looking to
“support her joints.”

Now, the clerk was clueless–he’s just trying to sell a supplement.
In fairness, he’s not really a health professional, so he’s really
not educated enough to correct her. Fact is, many people confuse the
needs of their bone with the needs of their joints.

They’re NOT the same, which I gently advised her, after explaining

Keystone Cop-Out!

The newswires all last night were abuzz with the latest fiasco to
come out of Washington. Despite all the good that would come out of
it, tens of thousands of jobs for a struggling economy, most of them
rank and file union jobs, the potential for energy independence from
the Middle East and regimes that hate our guts, a great joint venture
with our close and friendly neighbor to the north and a vast new
supply of oil that would drop the cost of gasoline for average Americans,
the Obama Administration is reportedly getting ready to VETO the
Keystone Pipeline project.

The excuse given is that under the “artificial deadline” imposed by
Republicans in the last debt limit raising agreement, “there is not
enough time to assess the environmental impact” of the pipeline.

Please!

They have known about this since 2008! Either they really ARE that
incompetent and have not done their job, by having that evaluation
completed long before now, or they just don’t want to lower the price
of gasoline for the average folk, to make the ridiculous “green”
projects, like the ill-fated Chevy Volt, or Solyndra, for that matter,
look better by comparison.

As for all the real jobs lost and the devastating impact on our economy,
hey, too bad! That’s just politics. The labor jobs lost? Well, they’re
just a necessary sacrifice to the cause of throwing yet another impediment
at the internal combustion engine.

Add this irrational insult to the “moratorium” on offshore drilling in
the Gulf that has so devastated the Gulf States’ economies and allowed all
that precious drilling equipment to migrate to Brazil and other sensible
countries, while CHINA drills horizontally off Cuba into OUR oil fields,
to siphon away OUR resources and you have to be deaf, dumb, blind and
stupid not to see what’s going on here.

The environmental whackos are probably orgasmic with joy…despite the
probablity that the functional impact on the environment would be close
to zero. In fact, we’re talking about a friggin’ pipe, for God’s sake!
It’s not like it’s a swathe of strip mining running through the country….

It’s all a slapstick farce…a Keystone Cop-out! And Canada is going to
get tired of being jerked around by these guys and send their pipeline
out to the West Coast, so they can sell their oil to China. And that
leaves us paying more and more for oil from the Middle East and other
unfriendlies and leave us perilously vulnerable to any upheavals there,
which seem to happen every half hour, or so.

Please forgive my rant, today, but these guys are so inept, behave so
contrary to the interests of the country, that I just can’t stand it!
Every so often, I just have to let it out.

Maybe some last minute glimmer of sanity might yet emerge, but I admit
to a certain pessimism, based on past performance. This IS the government,
after all.

And THESE are the guys that want to run our health care system. Are you
feeling a little uneasy about your future? You should be.

Look, if you take my advice, you’ll start taking care of yourself,
pronto. Eat right, make better choices for your meals, get rid of the
love handles by getting OFF the couch and MOVING. If you smoke, QUIT!
NOW! You can add about 20 years to your life, if you do.

Drink more water–most people are dehydrated. And if you begin to lose
fat, you will need even more. I recommend about 1/2 your body weight,
in pounds, in ounces of water a day. Start a walking program and begin
to do some calisthenics, or bodyweight exercises, or use free weights,
any form of resistance exercise, every other day, to build up your
lean body mass.

Relieve Knee Pain–Even With End Stage Arthritis

Here’s an email I received, just the other day, that asks about a common
knee pain problem. I get a lot of this particular question, so I thought
I’d pass it along to you, so you could benefit from the advice I gave this
lady. Naturally, her identity has been deleted to protect her privacy.

“Dr. Stillwell,

I live in Tennessee and am suffering from acute knee pain from a sports
injury I received many years ago. I had several MRI’s taken over the
years, but only the last one revealed anything at all: I have cartilage
now missing in half of my right knee. I recently turned 56 years of age
and partial knee replacement was not recommended for me 3 years ago (too
young). Injections were recommended to relieve the discomfort, which I
opted to do. The injections did not bring much relief, and what relief I
did experience was entirely gone in just a few short months. I haven’t
seen a specialist since.

Frankly, I’m afraid of surgery and having an artificial knee (even
partial). I know three people who have suffered complications from such
surgery (one eventually had his leg amputated above the knee, the other
friend now has a rod).

I have heard of natural injections where cartilage is injected, but
this is extremely costly (also not covered by insurance) and doesn’t
appear to work for patients who have lost so much cartilage to begin
with.

I acknowledge you are very busy and also don’t practice in your field
anymore, but I found out about you through the Hippocrates Health

Well this is an extremely common problem, especially for those of us
who are in the Baby Boomer demographic. We ain’t gettin’ any younger and
the aches and pains of osteoarthritis, the most common form of arthritis,
are a source of constant discomfort and worry.

Fortunately, there ARE things you can do that take advantage of your body’s
ability to heal itself, given the chance, and assuming the disease is not
yet “end stage.”

Here, then, is my reply.

“Hi Ms X,

Sorry for the delay in responding, but I don’t get around to this
inbox as often as I should. Also sorry to hear of your pain, which
sounds like arthritis. If that is correct diagnosis, then the
“cartilage injections,” which are actually two separate operations,
won’t work for you.

The injections might have been steroids, to reduce pain. But what
would work better is injections of hyaluronic acid (Hyalgan, Syn Visc,
or similar formulations). When they work (about 75% of the time),
they often relieve pain for up to six months at a time. And they can
be redone, to extend the functional life of the knee for quite a while,
to avoid surgery.

There are also braces that can “unload” your knee and shift weight to
the other, better side of the knee, where the cartilage is much better.

For end stage arthritis, a total knee replacement is the final solution.
All surgeries come with potential complications, but the risks can be
minimized by using a real joint replacement specialist. I don’t mean
just an orthopaedist, but an orthopaedist that specializes in joint
replacments.

You can find one by asking the local university hospital, or the local
medical society.

A total knee replacement (and even moreso, a partial knee replacement)
is very technique dependent. That means that your odds of a successful
outcome are improved considerably if you have surgery by a real expert,
who does the procedure all the time, with hundreds, if not thiousands
of cases to his or her credit. It cannot guarantee the result you want,
but it substantially improves the odds. I can’t emphasize this enough:
all surgeons are NOT created equal.

A Plan To Help Your Heart

The other night, I was watching John Stossel’s special on the Fox News
Channel. The subject was how many people’s beliefs are not borne out by
examination of the facts. Yet we make decisions all the time, mainly
on our beliefs, which are often in conflict with reality.

One example that jumped out at me was that the NIH (National Institutes
of Health) reported that the U.S. Government spends FOUR TIMES as much
funding cancer research as they do on research on heart disease.

FOUR TIMES!

Yet cancers account for 10-11% FEWER deaths than heart disease. And cancer
is only the SECOND leading cause of death, not the first, which is where
heart disease ranks–right at the top of the heap.

I’ve written here before about the chief characteristics of heart disease.
The most common first sign of cardiac disease is…SUDDEN DEATH!

Many people die suddenly, not even realizing that they had a problem
brewing inside them. And those who don’t die may have crippling chest
pain and shortness of breath and other signs of cardiac failure,
indicating that their vessels are narrowed to the point that they have
symptoms.

Usually, that indicates a narrowing of the inner diameter (lumen) of the
coronary arteries of 70% or greater. That narrowing progresses gradually,
over time, caused by oxidized LDL cholesterol deposits within the walls
of the arteries.

Narrowed (stenotic) cardiac vessels restrict blood flow to the heart
muscle, which is responsible for the deep aching chest pain of angina
pectoris, when the heart muscle’s demand for oxygen is greater than the
blood flow can deliver.

If, instead, that narrowed artery is suddenly blocked, by a clumping of
platelets, which clot off the remaining opening (thrombosis), everything
downstream is then suddenly deprived of oxygen. This causes the heavy,
crushing chest pain of a heart attack (myocardial infarction, or M.I.)
and results in dead heart muscle cells.

If the M.I. is sufficiently large, the heart may stop (cardiac arrest),
or it may begin a fatal arrhythmia, like ventricular fibrillation (the
actual cause of my father’s sudden death). In either case, blood flow
to the brain and other vital organs simply STOPS! And then, you die…
just like that!

Now, I realize that cancer seems scarier, because it seems to come
out of nowhere, for no apparent reason, and is a harbinger of a long,
painful and lingering disease, associated with painful and disfiguring
surgeries, radiation, chemotherapy and uncertainty, often culminating
in death.

No doubt, it can be a horror.

But heart disease is still the number one killer. And many of the measures I
recommend to prevent heart disease ALSO fight cancer.

So, if I were you, I would put my money on that, in terms of prevention.

Defending Oprah Winfrey

Hope you enjoyed your weekend. For me it was restful, but by Sunday,
it was time to replenish the larder. So off we went to visit Cosco.

We tend to go just an hour before closing, most times. It’s quieter,
the free samples are finished, eliminating the temptation to have some
and it’s more efficient, with far fewer souls remaining.

Up and down the aisles we went, scratching each needed item off our
list. Finally, we were ready for the checkout. My wife repaired to
the rest room, while I took care of the purchases.

As my items were checked off, the lady at check out saw my wife’s
“O” Magazine, a favorite fo hers. “Oh, look, Oprah,” she said to her
companion, who was packing my items into the shopping cart, as they
were scanned. “You know, she’s started her own network, now. Probably
making more millions.”

“Yeah,” said the first woman, a “as if she needed any more.” By this
time, I had finally reached my admittedly rather shallow limit.

I said, “You know, she earned every dime of her fortune and she’s
employed a lot of people and done a lot of good, along the way.”

The woman replied, “I guess that’s so, but still, at a certain point…”
Her voice trailed off and I realized she sounded exactly like a certain
party, who shall remain nameless, but who resides at 1600 Pennsylvania
Avenue, in D.C.

She resented the astounding success of this woman, who is entirely self
made. I was seeing a full-fledged case of unvarnished envy.

I would never make her understand how distorted, how un-American her
sentiments were, especially in just the few minutes I had. So I just
smiled and filed it away to think about.

This resentment of achievement is exactly what’s wrong with the country.
This kind of attitude was unthinkable when I was a boy.

Oprah Winfrey is a big lib and politically, not my cup of tea. But she
is also a brilliant, talented, altogether admirable woman who HAS earned
every dime she has and, as far as I’m aware, has never stolen from, or
cheated anyone. She started from nothing, overcame formidable obstacles
and created herself, with unparalleled business acumen.

She has been crtiticized in the past for how she chose to spend her
own funds, building a school in Africa, giving away cars to her audience.
Even though she seems unaware that her entire existence refutes her
political philosphy, I have no doubt she is a well meaning, kind and
generous person.

But all that aside, it’s HER DAMN MONEY! IT’S NONE OF OUR
BUSINESS what she does, or doesn’t do with it. It’s certainly not for
any politician to determine. None of it would exist, if not for her.

I’ve about had it with the current class envy sentiment fomented and
encouraged by the White House. From “Tax the Rich” slogans to “They must
pay their fair share,” when in fact, they pay FAR in excess of any “fair
share”–they, in fact, support the entire system. But it never seems
to be enough. And who is to decide what “fair” is?

I’ve always been struck by the irony that if such envious people spent a tenth
of the energy they waste in bitterness toward achievers, they could make
themselves better, if not rich. But such a concept is lost on the “Occupy wherever”
crowd… and this woman in Costco. But the fundamental irrationality and unjustice
of it place me in the odd position of defending Oprah Winfrey.

I see the same thing directed at people who are now attractive, who were once
fat slobs. They paid the price, they put in the effort. They achieved their weight
loss goals, against all obstacles. They deserve admiration and praise.

But there is always some fool who whines and thinks, “It’s not fair”
that they now look good and are far healthier than those who envy them.

It’s Never Been So Easy To Lose…

Just finished doing my webinar for Global Teleclass. If you were
part of my international audience, thanks for attending. If you
couldn’t make it, well, you missed a pretty good event, if I do
say so.

In fact, I had arranged to have the entire thing recorded as an
MP4 file, so it could be made into a DVD. However, Murphy was
working overtime on this one, so I’m afraid if you missd it, you
missed it.

First, last night in the wee hours, I was fine tuning my final
Powerpoint slides on the computer and had just finished. I was
literally just about to click the mouse button and save the
presentation in my Documents, when all of a sudden, Microsoft
invaded.

Out of the blue, with absolutely NO warning, the computer display
began to wink out, an icon at a time. Then a box came up, advising
me that Windows was installing some updates. The computer continued
to shut itself off, to my horror…and there was nothing I could do
to stop it. Terrific. I just hoped my work hadn’t been erased.

When it restarted itself and Powerpoint came back on, the entire
day’s work was just…GONE! Wonderful! Hours and hours, just
evaporated. Imagine my joy.

Well, I went to bed in complete disgust, got up early and started
over. I finished redoing all the slides about 15 minutes before
showtime. Nothing like cutting it close, huh?

The webinar itself went smoothly. I felt good about it and the
attendees and host seemed to like the presentation. So I felt
I would have a great recording to share with all of you who couldn’t
make the webiner, for one reason or another.

Right after it ended, I got an email from the lady who does my
audio and visual recordings. Guess what? The audio didn’t come
through, though the slides were recorded in silence.

Great. The hits just keep on comin’!

Although I’ve always wanted to do a primal scream, it’s been my
experience that it really doesn’t help much.

Besides, this is not that big a deal, compared to discovering I
don’t have the right size hip implant, after the nurses told me they
did, while the patient was open on the operating table. I’ve actually
been there, so I know. No comparison.

Well, like my old professor used to say, “Into each life, a little
rain must fall.”

So, I’m sorry to report that there is no recording to share with you.
But, I may redo this as a stand-alone video, because the information
is a great introduction to the fundamental principles of my popular

While shopping in the local Publix yesterday, my wife and I came
upon a young stock clerk who was very outgoing and cheerful. As it
happens, he was restocking the Greek Yogurt display, one of my
favorite sources for a high protein snack, or dessert.

If you don’t know, fat free plain Greek yogurt is sugar-free and very
high in protein, the best ones having about 23 grams of protein per
half cup, compared with only ten for regular yogurt. I like the brands
Oikos (which is organic), or Chobani (which isn’t, but is still very
good and a bit less expensive).

Mix a half cup, or even a full cup, of that with fresh blueberries
and strawberries and some sweetener, like Truvia, and you have a
very satisfying meal that tastes great.

It can also double as sour cream, if you take it with smoked salmon
or other dishes (like creamed cauliflower, as a potato substitute),
and done up in a blender, it makes a great smoothie.

Our happy clerk was not a fan of Greek yogurt, but then, he didn’t
know any of what I just told you. But now, he does and he promised
to try some of my recommendations.

Start Looking Out For Yourself

I spent all day yesterday and plan to spend most of today fine tuning
and tweaking my upcoming presentation for Global Teleclass.com, this
coming Thursday. You’d be surprised at how much work is involved.

All this stuff is right out of my head, because I know my subject,
but condensing it, to give the attendee the most information in the
shortest period of time, is what’s tough. It’s also difficult to gear
my vocabulary to my audience, so I’m well understood, but they don’t
think the concepts presented are too simplistic. It’s a fine balance.

I was on the phone today with a very nice guy, who will be interviewing
me and fielding questions from the audience, as a part of the webinar.
He was putting me through a dry run, to identify any areas that needed
improvement or further development and this was very helpful. I expect
this to be even better than my last webinar.

As we were chatting about this and that, he happened to mention a phrase
he’s noticed on current TV ads, promoting drugs to lower cholesterol,
such as Lipitor and the like.

The phrase was: “Get your cholesterol where your doctor wants it.”

Just think about that for a minute. You are being advised to do what
your doctor wants you to do. It is assumed that you don’t know where
your cholesterol level ought to be. And maybe you don’t. But maybe
you should…it IS after all, YOUR body, is it not?

Now, as a professional authority figure myself, I can appreciate the
sentiment (and the confidence) of the copywriter for the ad. After all,
physicians have spent many of the best years of their lives in endless
education and training, learning about disease and how to treat it,
so you don’t have to.

But as a patient, healthcare consumer and advocate for patients and
regular folks, I think the correct phrase should be, “Get your cholesterol
down to where YOU want it to be,” or, “Where it should be, for optimal
health.”

Especially with the continual erosion of medical care, whether from a
Leviathan government (Obamacare), or excesses of the private insurance
industry (HMO’s, PPO’s, outrageous escalating premiums, et al), YOU
must assume responsibility for yourself.

You can do that by educating yourself. With all the information out
there, especially on the web, there’s simply no excuse for total
ignorance.

Look, you can’t develop experience or judgment without some
instruction. That’s why it takes so many years to create a physician.
Remember, none of us was born knowing what we know now.

But you CAN learn about your body and the many options you have, so you
can make intelligent decisions and actively participate in your own medical
care.

Let’s take that cholesterol question, as an example.

Only a few years ago, the clinical target was a total cholesterol of less
than 200. Today, it’s an LDL (low density lipoprotein, or “bad” cholesterol)
of less than 100 (or below 70, if you’re a diabetic, or have proven heart
disease). And in fact, it’s become even more refined than that.

It’s now recognized that it’s the oxidized LDL that causes narrowing of
the coronary arteries, which are then subject to the sudden blockages t
hat result in heart attacks. And it’s also now known that inflammation
causes the attraction of platelets, which clump together and cause the
“white clot” that closes off the already narrowed vessel.

There’s no reason you can’t know any of this. The concepts are not
difficult, if someone explains them to you. And that’s exactly what I had

to preserve your life: mainstream, alternative and complementary.

While creating my serial “slides” on Powerpoint, I remembered with great
fondness and respect, the guy who taught me how to give public
audiovisual lectures.

Years ago, when I was just starting out on the lecture circuit, my Chief &
Professor, “Wild Bill” Fielding, Chairman of Orthopaedics at St. Luke’s

Fielding, though, was in a class by himself. He had electronic “lap
dissolve” equipment, so he could produce cool effects like one slide
fading out, as the next was fading in. Today, with computers, that’s
no big deal.

But in those days, that little trick was a major “tour de force,”
requiring two slide projectors on each side, synchronized and “zeroed
in,” so the images were exactly matched. Like I said, a major production.
And he was a virtuoso, without peer.

Today, I can make far better, clearer digital “slides” right on my P.C.
than I could ever have made with photography, as I used to. In fact, the
hardest part is boiling the subject matter down to its essentials, so
my viewers will get the best, most concise concepts, without a lot of
fluff.
