Worked my butt off this past weekend, cleaning the house for an anticipated visit by my father-in-law’s wife’s brother and his wife, from Italy. Seems the guy is reported to be allergic to cats. Heh. That would make ours the wrong house to visit, as a general rule (we’re down to 24 now, having just lost Misty, our oldest, on this past Christmas Eve Day).
But, as these nice folks have never been in the States before, in the interests of international diplomacy, we decided to go the extra mile and super clean the place to make it “safe” for an allergy sufferer.
But first, we had to trap the cats and confine them. Otherwise, you’re just spinning your wheels. Fortunately, like me and most of our friends, they’re “food sluts.” Nothing they wouldn’t do, no place they wouldn’t go, for a tasty meal. So, my wife and I cracked open a few cans of wet cat food and played “Pied Piper” with them.
It’s quite a sight to see them all following the aroma of the food, right into the bedrooms, where we locked them in for the day. Then, we could work in peace.
So, we finished the house and then set about making some food. One thing about Italians is that they know good food. So we pulled out all the stops. Turned out pretty well, too. They seemed to like everything and had seconds….
Nice couple they were: Mario and Tina. Live near Naples (the real one, not the one in Florida), have four kids, the oldest of whom is now in his forties. And they speak no English. Fortunately, Anna, Charlie’s wife and Mario’s sister, is bilingual, so we had our own built-in interpreter. Very lovely people. And we had a good time.
During the meal, Mario mentioned to my father-in-law that his son was supposed to have surgery on his knee. Between Anna’s interpretations and sign lingo, I was made to understand that he was having arthroscopic surgery on his lateral mensicus, which according to an MRI scan, was torn. And, since he found out that I was an orthopaedic surgeon, he naturally wanted to know if I thought that surgery was necessary.
Pretty good question, really. I pointed out to him that if he had read my book on treating knee pain, DR. And a lot has to do with the nature of the tear, where in the meniscus it is, and how active he intends to be after surgery. And all that is assuming that the tear actually exists.
The MRI scan is a great tool for diagnosis. On the medial side of the joint (inner side), its accuracy is about 90-95%. But, because of the screwy anatomy of the lateral compartment of the knee joint, MRI findings of the lateral meniscus are only about 60% accurate. So, actual pathology has to confirmed at arthroscopy, before you can begin to decide on appropriate treatment.
Apparently, he got the injury playing football. Not our idea of football, as in the Superbowl, but European football, or soccer. The rapid twisting, cutting movements in that game result in lots of meniscal tears. And, despite his age, he’d like to continue playing, after the proposed procedure. That being the case, he SHOULD have the surgery.
If he was a low demand user, ie., a couch potato, then the same injury need not be treated so aggressively. Then, it was time for them to make the run to the Orlando International Airport. Another of his sons was flying in to visit and they were going to pick him up. Naturally, they all want to go to Disney World.
So, having done our bit for U.S. international relations, and made two new friends, we all said “Ciao!” and bid them farewell. After a few well-earned moments of peace, for ourselves, the bride and I released the feline hordes, who descended on the kitchen, looking for more food! There’s no end to it!
And, you know, that guy, Mario, never sneezed once! Guess we did a pretty good job. Well, that’s it for today, my friend.

I’m back in Southampton, having flown in yesterday. So this past morning, we drove to the airport to pick up my father-in-law and his wife. We thought it might be nice to gather at the new house for Thanksgiving and to see in the new Christmas Season.
And to be honest, there’s something bizarre about having Thanksgiving and the Christmas Holidays in 70-80 degree temperatures in Florida, when you’ve grown up associating them with the cooler weather of Fall in the Temperate Zone. You know, the whole “chestnuts roasting on an open fire” thing rings truer when “Jack Frost is nipping at your nose.” When it’s perrenial summer, not so much.
So, to prepare for the Season, my wife got us tickets to Winter Wonderland in New York for later this afternoon. True, the Rockerfeller Center Christmas Tree won’t be lit yet; the official Christmas Tree Lighting Ceremony won’t be for a couple of weeks– I think it’s December 4th. But all the other Lights and Christmas Decorations will be up and well worth seeing.
So, I was prepared for cooler weather and looking forward to it. But last night, my wife and I went out shopping, and man, it was COLD! Late last night, it was down into the twenties. I guess I’ve been in Florida too long. I was looking forward to the cooler weather but here I am, freezing in Southampton!
Well, hopefully, it will warm up a little later. Brrrrrrrrrrr! This part of New York I didn’t miss.
Funny thing about cold weather is that, like damp weather, it can make your joints ache when you have any degenerative joint disease. Among the things that can make you feel better and provide a little relief are supplements and joint wraps.
I’ve mentioned before that if you improve the balance of Omega 3 Fatty Acids in your diet, by avoiding red meat, saturated fats and other sources of Omega 6 Fatty Acids and then taking fish oil and other sources of Omega 3′s, you can reduce inflammation and therefore, relieve joint pain.

Let’s face it. The internet is a gusher of information: some good, some bad, some irrelevant. So much, in fact, that the average person is just swamped–people have NO frame of reference, no filter to determine what information is correct and what isn’t. What you really need is ADVICE, from someone who KNOWS. And that’s what I’ve tried to provide.

Well, here we are in Orlando, where it’s cloudy but not even raining yet. And the press has worked itself into a lather over the approach of Fay, a tropical storm with hurricane potential. Now, it is true that Fay has spawned some tornados, apparently killed ten people and should be taken seriously. But so far, virtually nothing that has been predicted has happened.
I’m well aware that meteorology is an inexact science. That’s rather like saying King Kong is a large monkey. I mean, these guys are wrong so often that they could probably do better if they flipped a coin. They know it’s raining when they look out the window. For all the sophisticated computer models, specialized radars and scientific analysis, they are clueless about what’s going on in the real world.
See, the problem is the computer models. The models are accurate ONLY if the premises on which they’re based are valid. Wrong assumptions, wrong results. Since the facts on the ground (or rather, in the air) are always changing, it makes it tough to accurately predict what’s actually going to happen. You see, I understand their limitations, but talk about your overpaid guys! They get paid whether they’re right or wrong! In fact, most people expect them to be wrong. Meanwhile I was supposed to be perfect–any deviation from perfect is assumed to be defacto evidence of malpractice. Feh! Makes me sick!
Don’t get me wrong. I’m glad we don’t have to deal with a major hurricane. It’s just annoying that every prediction these guys make is presented with a certitude that keeps us all in a state of fear. That was the premise of a very good book by Michael Crichton, he of Jurassic Park fame, of the same name (State of Fear). His theme was the constant “state of fear” that the media tries to keep the public in, with one dire prediction after the next. And it’s absolutely true.
And here’s something else to think about: this is the same “science” that supports those who buy into the whole apocalyptic “global warming” business. Almost all of it is based on computer models, just like the day to day weather, apparently with the same degree of accuracy.
This would explain the phenomenon of melting polar ice caps, rising seas (which, given Al Gore’s time line, should have swallowed several coastal cities by now) et al, which are a product of extrapolated computer models. Same problem: if the assumptions on which the models are based are faulty (ie. warming, such as it is, is the product of man made activity, greenhouse gases and the like), the results are, too. This is why the polar ice caps of Mars are also observed to be receding, absent ANY man-made activity (so far as we know). And it explains why our polar ice is now INCREASING, contrary to the predicted findings of the models.
People who are fully invested in the whole global warming enterprise are aware of these disturbing variations in the real world from the computer models. That’s why you now hear more about “climate change,” than global warming these days. Here’s a clue: the climate is ALWAYS changing–that’s what it does. And all climate is a function of the activity level of the SUN, the source of all energy on earth. The amazing thing is not that climate varies now and again, but that the narrow range necessary to preserve life on our little planet is remarkably constant.
So while the meteorologists and weathermen appear to be fey on Fay, I’m going to look out my window, onto the real world, and see what the weather’s doing. Still cloudy, still no rain, mildly breezy here in Orlando. But the radio says twisters were noted in Southern Florida. Which way will it go? They have NO IDEA, though they have plenty of predictions. Ah well, let’s hope it misses us. We’ll soon know…
Speaking of the crappy weather of the Hurricane Season, ever notice that your joints ache when it’s about to rain? Or that you feel sleepy when it’s cloudy and damp? Well, that’s a function of specialized nerve endings in the tissues around your joints that respond to a fall in barometric pressure (baroreceptors). The phone rang the other night and I almost missed it. It’s a big house, so if I’m not right next to the phone I have to run to the nearest one, without running the cats underfoot. But, I got it just as the answering machine picked up. Then, I had to wait for the recorded message to finish before I could hear and be heard.
It was one of our good friends, who is a female cop. She’s a sweet girl, but she’s one of those people I don’t hear from, until she’s got some medical problem. I have more than my share of those. But, hey, it’s good to hear from them, regardless of the reason they decided to call.
It seems she was playing racket ball and suddenly felt this lancing pain in one of her calves. Because it was this sudden, hot poker sensation, she was a bit scared. Who could blame her? She had heard of painful clots in the calves that could be dangerous (deep vein thrombosis), so she was pretty anxious.
Well, I asked her a few questions, so I could try to figure out what had happened to her. You may not realize this, but it’s tough to diagnose anything, sight unseen, over the phone. Lay folk simply don’t share the same vocabulary with medical people, so it’s hard for them to accurately describe what they feel, or what actually happened, or even what something looks like. So, pity the poor doc who’s trying to make sense of what he’s being told and trying to make an intelligent diagnosis, based solely on this questionable information.
I’ll give you a great example of what I mean. My father-in-law used to tell me that the outer part of his hips (the trochanteric region) felt “numbish.” Now, to a physician, “numbish” indicates a decrease in sensation; that’s what “numb” means to us–a possible neurological problem. What he actually meant was “dull pain,” which indicated a bursitis, but he didn’t know how to accurately describe it to me. Took me a while, and a physical examination, to figure out what he really meant. The point is, I had to interpret what he said into what he really meant. So, when you’re trying to do this over the phone, it just adds another layer of difficulty to a tough situation.
Anyway, this sharp, hot pain at the back of her calf was familiar to me. I asked her if she felt a “pop” when she felt the pain. “Oh, yeah,” she said, “now that I think about it, I DID kind of feel a “pop,” when it happened.” I also found out that she was able to walk on it, even though it hurt, and that her calf swelled up and was tender. I should mention that she’s in her forties, now, too.
Assembling all the pieces, I felt pretty confident that she had experienced a rupture of the plantaris tendon. Given her symptoms, her age, the activity she was engaged in and my past experience with these same symptoms, it was most likely that she had popped her plantaris tendon. So, I first reassured her that she most likely didn’t have anything really bad, and she probably wasn’t going to need any surgery (for a discussion of this and other posterior knee conditions that can be treated without an operation, please see my newest healing program HOW TO AVOID KNEE SURGERY http://drbillsclinic.com/avoid_knee_surgery.html )
Then, I explained what had happened to her and why. And then, what to do about it. Once the tenderness and swelling were gone, then she could begin the stretching and strengthening exercises taught in my PAIN-FREE PROGRAM, which you can see at http://drbillsclinic.com?exercise_eliminate.html The good news is that she’ll be back in action in a few more weeks with no disablity. And she won’t have any residual weakness.
Of course, she could still rupture the OTHER one…. But with the proper warm-up before any exercises, and the recommended stretching and strengthening, hopefully, she’ll avoid it.

My wife picked me up at the airport the other day, after my flight to New York. It was good to see her. Especially since I had worn a green polo shirt, in honor of St. Patrick’s Day and it was COLD outside the airport, if you’re used to Florida temperatures. Once inside the nice, warm car, with the butt warmers on, I settled in for the road trip to Southampton.
While we were catching up on events, I noticed that she was squirming in her seat. I asked what was the matter and she said “My butt is sore.” Turns out that she’s noticed this just the past few days, just about the time that her new car was delivered and she started driving it. Hmmmmm….
Well, further questioning revealed that what was actually sore was her right greater trochanter. It just seems like her butt to her because the trochanters, the hard bony lumps at the outer upper part of the thighs, that lay people often refer to as their “hips,” actually roll backwards, toward your butt, when you sit. To make matters worse, and what was probably the cause the onset of this pain, the new seats have a deep “bucket” profile, moreso than the car she’s been driving, til now.
So the side walls of the bucket seat press in on the trochanters, which roll backwards, right into the “bucket.” This extra pressure rubs the bursa, a sac of fluid that normally lies over the prominence of each trochanter to allow gliding of the soft
tissues over the bones. This extra pressure causes inflammation, a bursitis. Once started, the bursitis is worsened by any additonal pressure or friction.
If you develop trochanteric bursitis, for whatever reason, there are things you can do to relieve your symptoms. First, avoid that activity that’s aggravating the symptoms. In this case, a pillow or gel pad in the seat can cushion the trochanters and raise the level of the seat, to eliminate the sidewall contours of the bucket seats and the pressure they apply. Next, rest and sleep with the legs apart, to relax the soft tissues over the bony prominences. Often the best way to do this is to place two or three pillows between the knees. Finally, you need to reduce the inflammation, by using OTC medications, Advil or Aleve, applying local heat, and using the various alternative measures. Oh yeah. One more thing–while you’ve got pain, use a cane in the opposite hand. This will reduce the pull of the hip abductor muscles on the tender greater trochanter and allow it to heal more readily. I’ve not only treated many people, besides my wife, for this condition, but I’ve had it myself, as well. So I can tell you, first hand, that these measures work.
